If you have already received a date for your appointment, here is some important information.

 

If you are unable to attend your appointment, please let us know as soon as possible. Contact Us, Email us at drgoldade@gmail.com or text 403-620-4204 so we can arrange an alternative.

 

  a) Between 2-4 weeks prior to the appointment, you will receive a set of questions from the Ocean Patient Portal (from no-reply@cognisantmed.com) that we ask you answer to help us prepare for your visit. Please send us the answers well before the appointment.

 

  b)  7 days prior to the appointment, you should receive a confirmation email from the Ocean Patient Portal (from no-reply@cognisantmed.com) to which we ask you to reply. Please confirm your appointment as soon as possible to allow others the opportunity to use the time slot if needed. If you do not reply, you will continue to receive email reminders 5 days prior and 3 days prior.

 

  c) Between 2-5 days prior to the appointment, an email invitation will be sent directly from Telus Health (no-reply@virtualvisit.telushealth.com ) with instructions and a LINK for the appointment. It is sent to the MAIN (one) email we have on file. You are welcome to share the link with others so that they can join (a second parent, the child/youth, a teacher). Do not DELETE the email.

    Note that sometimes the email from Telus goes into a Spam Folder. So, if you can't find it, check there.

    You will need an Internet-capable device equipped with microphone & camera (computer or smartphone or tablet). You do not need to download an App.

    And note that some work computers might block the Telus link.

    Ensure your browser is compatible. You may need to use a different browser.

    Click on the link to ensure it works.

    Ensure your camera and microphone are working.

 

  d) **** Be sure to obtain your child's Height and Weight before the appointment. It's up to you if you wish for your child to attend, but know that Dr. Goldade always likes to *see* the kids.

 

  e) Five minutes before your appointment, please click the CONNECT button. You'll be asked to give access to your camera and microphone, so that Dr. Goldade can see you and speak to you.

 

  f) If there are any technical difficulties at the time of the appointment, please TEXT 403-620-4204 or send an Email to our admin at drgoldade@gmail.com to resolve any issues. Do NOT Phone.

 

  g) Feel free to send info (Psychoeducational Assessments, Report Cards, IPP/LSPs, Letters from the School) before the visit via the Ocean patient portal.

 

  h) If the issues/questions for your child involve ADHD or Behavioural or Mental Health issues, please have each parent, the child (if over 10 years of age) and the teachers/coaches/tutors complete any relevant rating scales via:

  i) For any acute Mental Health in the meantime, consider going to The Summit.

 

  j) Note that Dr. Goldade is now using an AI Scribe to record the Virtual Visit and then an encounter note is created for the file. If you are not in favour of this recording, please let us know.

Please give us LOTS of time to cancel/reschedule as many people are eager for the appointment spots. You may be billed for appointments cancelled with less than 24 hours notice, as well as any missed appointments.
